- Combined Behavioral Health Assessment/Comprehensive Diagnostic Assessment (CDA) with skip logic over modules not relevant:
- Mental Health Assessment (optional)
- Substance Use Assessment (optional) including ASAM criteria
- Treatment Episode Data Set (TEDS)/federal data collection
- Consent to Release of Information/Authorization to Use and Disclosure (AUD)
- Person-Centered Services Plan (PCSP)
- Social Determinants of Health, Health Leads (SDOH-11)
- Columbia Suicide Scale (C-SSRS)
Youth Empowerment Services (YES)
- Idaho Child and Adolescent Needs and Strengths 3.0 (CANS) – requires certification
- Scoring for Tiers 0, 1, 2, and 3
- Scoring for consideration of High Fidelity Wraparound
- Plan of Care for High Fidelity Wraparound, meeting YES Principles and Wraparound Process
