Available to providers via Availity Essentials

All credentialed providers have access to outcomes and assessments applications through secure single sign-on via Availity Essentials (Magellan Healthcare of Idaho Payer Space > Assessments tile). 

These assessments by population include:

All populations

  • Combined Behavioral Health Assessment/Comprehensive Diagnostic Assessment (CDA) with skip logic over modules not relevant:
    • Mental Health Assessment (optional)
    • Substance Use Assessment (optional) including ASAM criteria
    • Treatment Episode Data Set (TEDS)/federal data collection
  • Consent to Release of Information/Authorization to Use and Disclosure (AUD)
  • Person-Centered Services Plan (PCSP)
  • Social Determinants of Health, Health Leads (SDOH-11)
  • Columbia Suicide Scale (C-SSRS)

Youth Empowerment Services (YES)

  • Idaho Child and Adolescent Needs and Strengths 3.0 (CANS) – requires certification
    • Scoring for Tiers 0, 1, 2, and 3
    • Scoring for consideration of High Fidelity Wraparound
  • Plan of Care for High Fidelity Wraparound, meeting YES Principles and Wraparound Process 
  • Person Centered Services Plan (PCSP), which is a subset of the Plan of Care, including the Child and Family Team

Adults with complex needs

•    Adult Needs and Strengths Assessment (ANSA) – requires certification

Substance use screening (optional)

  • Drug Abuse Screening Test (DAST)
  • Alcohol Use Disorders Identification Test (AUDIT)
