As a reminder, providers serving clients funded through the State Opioid Response (SOR) grant must complete the SAMHSA Unified Performance Reporting Tool (SUPRT) forms for all SOR-funded clients. The SUPRT tool replaces the former Government Performance and Results Act (GPRA) survey and is required for federal reporting on grant-funded services.

Complete the SUPRT-A form at the required assessment points:

• Baseline (at the start of services)
• 6-month reassessment
• Annual (for clients still receiving services)
• Closeout (when services end)

During the current interim process, submit completed forms to MagellanSUPRTForms@MagellanHealth.com for processing and entry into the federal SPARS system.
